random radar icon for some of my pics?

Hi, when I do bulk upload on my gallery, I keep getting this icon that looks like a round radar image on some of my pictures and when I go to find out why it occured, the images have disappeared, as if it deleted them for someodd reason... Why would it be doing this? How can I prevent it? I don't see it mentioned in the docs or faqs.

What exactly does that mean? It doesn't appear again if you select the same folder in the mini browser or it doesn't exist on the FTP server?

if I load up the direct url of the image, it will return a 404 and when I look at the file tree, it's nowhere to be found.

Does it always happen to the same pictures (if you re-upload them, after they got deleted)?
