captcha failing as is. Can we add a picture fix?

The captcha feature is failing badly on the registration. I have almost entirely spam registrants.

I would like to see a thumbnail beside the registration that I can upload a small image. The registrant would have to type the right word depicted.
I envision a line in the config control panel where you can put the url of the image. To make it easy, the captcha-esque word would have to be the title of the jpg. The registrant would have to type the word or fail. I could change it whenever needed quickly and easily. I could put custom text on the image, or alternatively would like an input or  drop down box with "What x is depicted?"  with x= first name, last name, animal, sport, colour, etc and an owner entered answer would be nice too.
