Unwanted automatic redirection without trailing slash

Coppermine was installed successfully, but http://theflooringmill.com/coppermine (without the trailing slash) gets automatically redirected to http://theflooringmill.com/flooringmill.com/coppermine/ (note the extra directory).  http://theflooringmill.com/coppermine/ (with the trailing slash) stays the same, as desired.

I've looked around, but couldn't figure out why it's getting redirected.  I've tried the following things:
  • Reviewing settings in the Config menu
  • Looking at database entries in cpg_config
    • Nothing is set to flooringmill.com
  • Checking for modifications to any web.config files
    • No modifications are present

I successfully created a redirection in my web.config file that manually redirects http://theflooringmill.com/coppermine to http://theflooringmill.com/coppermine/ (which I have disabled so you can see the unwanted redirection), but would prefer change whatever setting is causing the undesired effect in the first place since there's obviously some kind of redirection going on.

Help and advice on this would be greatly appreciated.

Coppermine doesn't do that. It's something else you have set up.

Thanks.  I talked to my hosting provider, and will post their answer for completeness since it doesn't seem uncommon.
