my coppermine being hacked..?

Dear all;
Seems like my coppermine beeing altered/hacked, the header file has been altered (themes/themename/template.htm), see attachment.
Anybody have the same experience? anyone know how to prevent this?

Unfortunately it could be a number of things. You should check this thread to see if any of it applies to you

Along with coppermine, you may have other applications running on your server which may also compromise your install. Speak to your webhost and they should be able to give you more information from the logs.
