The previous file could not be placed (upload problem)

I'm now getting a msg that says "the previous file could not be placed"  I've tried a number of things to no avail. 

I've turned on debug info for all and attached the debug info for all the upload steps.  I've created a guest account:  username: test user  pw: password

any assistance would be appreciated.

First of all: upgrade, you're running cpg1.4.16, while the most recent stable release is cpg1.4.19!
Then do exactly as suggested in the docs, section "asking for support on upload issues". In your case: set the number of file upload fields to 1 and the number of uri upload fields to 0 for the group the test user account is in. Turn debug_mode on for everyone. Then post back here.

Thanks for your response.  I have upgraded and followed your instructions.

The test account produces the following response upon upload:
Unable to create thumbnail or reduced size image.

My account produces the same as before:
"the previous file could not be placed"
