Many people have some problems with captchas (not with the script), because there are often unrecognized elements on the captcha  (its also hard for people who have bad eyes too). Furthermore, one should not forget that many programs can ‘recognized’ elements on the captcha and can also send spam to your website. It is clear from these observations that captchas must be complex so that only humans can read them. In view of these facts, it is quite likely that it will be harder to cognition elements on the captcha. Moreover it can't help against picture/content grabber etc.. Content/picture grabber copies your web content and use them for their homepage/gallery. They dispose special programs which save your pages or/and your pictures on their local hard disk. One thing to check is if the spider respects robots.txt. Nobody will deny that criminals use programs which don’t use robots.txt. Whether they do not respect robots.txt, then you should use this program:

http://code.google.com/p/cpg-dnsbl/ (I used that and get only one spam comment every three days). It blocks bad robots.
http://www.bad-behavior.ioerror.us/ (English)
http://www.spider-trap.de/en_index.html (English/German), very good

And you’re right you will have less traffic because all bad robots (and that are many every day) can’t enter your site. Search engine robots like Googlebot or yahoo [bot] can enter your page without problems.
If the program think that you are an computer or your IP is on the black list you can also enter the site, but you must first solve an arithmetic problem or enter a captcha code. Look here for an example: http://www.spider-trap.de/spider-trap/forbid.php .
What can I do if somebody had stolen my content/pictures? You should first write an e-mail to the webmaster (not hoster). Please make sure that your content is not used under “Fair Use”. You can find more out here: http://en.wikipedia.org/wiki/Fair_Use . If it is not “fair use” and the webmaster do not reply immediately you should contact the host (not the domain registrar). Go to http://whois.domaintools.com/domain.com and enter the domain in place of ‘domain.com’ . Look at 'Server Data' and click on the red ‘W’ button next to the IP address. Then you send should send an e-mail to the abuse@..... . … address. At American/Canadian webhoster you must often write a DMCA-Notice to identify the material ( http://imajr.com/DMCA.aspx ).

So I installed http://www.spider-trap.de/en_index.html . At the moment I don't get spam.

1. Click on
http://icmesa.org/   (it will work)

2. Then enter:
http://icmesa.org/elizabeth-montgomery/index.php (Don't type in the captcha)

3. And then go to
http://icmesa.org/   (it won't work unless you enter the captcha )

This should be marked as Spam. When I enter to the site the word "SEO" appears a couple of times, so just in case, I deactivate the link.

If you link to your site again without doing a contribution, you'll be considered as Spammer and will be banned immediately.
