While browsing some Coppermine based sights I noticed a few that had a form on the intermediate image page that include fields for Image Number, Image Name (which was pre-filled with the file name of the image)

The forms appeared very similar on the different sights so I am assuming that it is a common plug-in or add-on.  I have done several searches ( with terms like email, image request, form, information request) and browsed the Mod/Add-on forum but I just don't see what I am looking for.   

Does anyone recognize this form?  Can you point me to it (if it exists)?

I should add that this was an email form and included fields for name and email address and comment (although it was not the standard image comment )

I hesitate to single out a link because these are not my pages but this google search:

"powered by coppermine" wildlife photo gallery "Image Name" comments

will produce numerous links on the first page that show the form I am talking about.
