The previous file could not be placed....MESSAGE

TWO issues:My photos are GONE replaced by an "X".

Upon Uploading attempt, I receive the following-- "UPLOAD Successful"...then I receive the following FYI--

The previous file could not be placed.

You have successfully placed all the files.

How do I "fix" this? Please advise.  :)

Do exactly as suggested in the upload troubleshooting section of the docs.
