Image changing after uploading it.... Weird..

Ok, so when I upload my images it always changes... I select one image and when it shows the image where you put a description for it, it's always some OTHER image from my files. It just won't upload the one I choose, on certain images. Others(a select few) do upload right, but most don't. I didn't see anything to that addresses this problem anywhere in the documentation.

Also whenever I try to upload an image with a link, it says, "Error Message: Could not open URI." everytime!
