"The previous file could not be placed."

I'm trying to add files (for the first time) and I keep getting this error: "The previous file could not be placed." Any idead what might be wrong?

Okay, now I get:

The file 'albums/userpics/10001/av-1039~0.jpg' can't be inserted in the album

Unable to create thumbnail or reduced size image.

File: /home/rayhne/public_html/gallery/db_input.php - Line: 391
