Comment fields - add email or name field - how?

Wondered how I could change what kind of information can be left in comments.
For example, I'd like to have a Name field, perhaps a website and/or email field as well
when someone adds/leaves a comment.  ???

Help on any additional small codings such as this would be appreciated.  :)

currently not possible - this would require additional database fields and a lot of changes in the overall code. This is not an easy task, not at all.

Then I have an example gallery of someone who did get it done if you'll permit me to share:


As you will see, in the 'Add your comment' fields, there is a space for a Name there, as mine does not and I am wondering how to perform this.
I have CPG 1.3, is this supported in CPG 1.4?

That name field displays by default if the user is not logged in and you allow comments by unregistered users.

It doesn't display if you are logged in or a registered user as CPG will automatically read the data from your profile.
