Title: The previous file could not be placed.

I see other messages with this subject. However, I cannot figure out what the solution is. My users (and I) were uploading successfully through March 10. Now we get the previous file  could not be placed message. I have followed the instructions in the document on troubleshooting this. What now?

No, you have not. Quote from that section of the docs:

I see other messages with this subject.
Then you should be aware that we told users who reported this error message exactly what I had to tell you. RTFM (with an exclamation mark).

You haven't done as suggested: the number of file and URI upload fields is not set as suggested. Don't play games with us - RTFM and do exactly as suggested there. We can expect you to make your homeworks before asking for support! >:(
