I cannot see a line with two green check points in your screenshot.

Please post more information about your server and/or gallery. Has it been upgraded? Do you use any plugins? Do you use a custom theme? Have you modified anything? Something special about your file or folder names?

Just a quick response in here, in case anyone has the same problem. I found the solution in duplicate codes of Javscript in template.html. So make sure you double check the commands on those.
