Here is a clean little template I have finally gotten right.

The header image is named header.jpg.  Size of the header image must be 940px wide by 198 px high.  Image is saved as /images/headers/header.jpg.
I have moved the file information on the intermediate page from below the picture to right of the picture.
I have also truncated the album description on the album list page and included the entire album description above the thumbnails on the thumbnail page.  For a family style site, this allows you to include your story on the same page as your photos.

Thanks to everyone who answered questions along the way while I was working on this.

That function needs to be removed from the theme.php. Only functions that are defined in include/themes.inc.php can be overridden by a theme.

Please try the .zip attached to this message.

I removed the mod to show the pic info on the right of the intermediate image, and moved the function back to its rightful place.

Please let me know if it doesn't work for you.

Looks a bit better... But no Image is displayed. Only a blank placeholder...
