Showing wrong thumbnails

Recently I had the iframe attack on my coppermine and I upgraded to 1.4.18.

Now if I upload a new picture to an album, sometimes, it will not make a "normal_" image, but it does create the file "normal_", but it is 1px x 1px !

Also pretty random, it will show the wrong thumbnail with the picture in my album list.

The cat picture is a test, but it shows the chart at the bottom :?

I am a little confused, I tried debug mode and I am logging, but I don't get any specific messages in the log or debug.

Review your config settings - the hack changes some of them.

I checked the settings, but I have no clue what could be wrong, it happends very randomly. Do you perhaps know what kind of settings I should be looking at?
