How to modify SYS_MENU or SUB_MENU (including group visibility)

Hi,

Can anyone tell me the definitive way to modify SYS_MENU and SUB_MENU, including setting the visibility per group (so a logged in user sees different custom menu items which are not visible to those not logged in)

The current "stickied" thread is incomplete in this regard
