Doin my head in the thing uploads and then this

This is really puzzling me as its letting me upload the file but then when i clikc ok i get this

The previous file could not be placed.

You have successfully placed all the files.

but it actually uploads it into the directory but doesnt show it on the actuall gallery


any ideas folks
