Changing the Default Form Field Names

This is exactly what I would like to do:

I know this is probably a longshot, but would anyone be able to tell me step-by-step "idiot-proof" directions on how to accomplish this, or is this something I would have to hire someone to do if I have no PHP knowledge? I did not set up the gallery and I do not know how to access the "lang/english.php." Is it something I can access as an administrator?

In your host file manager you have the edit option for files.
Edit lang/english.php.
Find File Title and File Description and replace with your custom text.
