Where do I put the icon image for a new menu item?

I'm getting this code show up when I put in a new menu button that links to my bridge smf forum using the custom menu link in configuring the theme...

No, I just found the {CUSTOM_LNK_ICO} in the template.php file and just deleted it, so it doesn't have any icon.  Its the only way I could figure out how to do since I was having problems view the template.html file.  When I get more time, I'll probably go back and fix that, but it works for now.
