No Files! Uploaded Error

Hello!
I have been using coppermine for two years! but never faced this type of error. It's displaying with my 4 galleries for last week or so! hosted on different servers.

When i upload any file using normal upload! it doesn't show thumbnails while ask me to save on which album but when it completed the last message shows like that

Information
The previous file could not be placed.

You have successfully placed all the files. 
CONTINUE

Please! tell me what error it should be, i tested some other galleries hosted on different showing same problem again and again!

If you have issues with uploading, do exactly as suggested in the upload troubleshooting section of the docs and the next section there that is named "asking for support on upload issues". Failing to do so will result in your support request getting ignored.

I have the same problem.
One issue per thread. Don't try to hijack someone else's thread. Your issues may or may not be related to the thread starter's issues.

Split posting by anacoelho into a thread of his own. Split the reply by WhiteBear into a thread of his own.
You're causing extra moderation effort. Don't reply to this thread, but to the one I created for you by splitting.
