Hi one week ago more or less i updated coppermine version to 1.4.5 and now, i cant view much old pictures.

examples:

i try uploading again the picture and in the first moment i can see but if if a refresh i cant view again.
Its posible a trouble with fields in white, and the Underssscore ( _ ) ?

The thing is, you are not using underscore, but spaces. However, that is not the issue since the thumbnail files work fine.

Removing thumb_ to get to the big file results in a 403 error. http://www.fotosdonosti.com/albums/san_sebastian/Imagenes%20Blanco%20y%20Negro/Arena%20la%20concha%20sin%20color.jpg

This is not a coppermine issue. Check that the file has the correct chmod permissions.

i do the change to cmhod 755 but its the same

Now i have this files in three times

zurriola_kursaal_059.jpg
zurriola_kursaal_059~0.jpg

and zurriola kursaal 059.jpg , this one i have update now and its working.

try chmoding the files 644. If that doesn't work ask your webhost about 'chown'
That's what happens if I try to access one of the mentioned pics directly --> You don't have permission to access yadda yadda yadda

so as TranzNDance already pointed out this is a permission problem (chmod, chown) and not a coppermine issue

and yes, if you upload the same pic twice, then they get numbered (~0, ~1 etc)

I dont know why, but when i upload a new picture, the pictures who was in gallery from older version (cpg 1.3.2). change the name. And all sepparations become This " _ ".
Why this? what can i do to upload picture and no " lose " older ones? ( i dont lose only change the name and cpg dont find they).

It's a coincidence. If you clicked the links Stramm and I posted, you get an error that is not related to Coppermine. So even if you uninstalled Coppermine and left the albums folder, you will still have the same problem. Ask your webhost. We can't help you anymore than your webhost could answer Coppermine code questions.
